Advanced Wound Care Qualification Criteria

To qualify for care, a wound must be at least 30 days old and classified as chronic. Types of qualifying wounds include Pressure Ulcers, Venous Stasis Ulcers, and Diabetic Ulcers.

Collaborate with nursing facility staff to treat eligible in-house wounds for free for both the home and the resident.

REFFERAL PROCESS

Send WoundSwift weekly wound reports for all residents

  • WoundSwift will review all wounds to identify wounds that meet the skin sub qualifications

  • WoundSwift will obtain an Insurance Verification Request (IVR) to secure payment 

  • Once all these steps are complete, WoundSwift will schedule skin sub treatments

  • Dr. Holly Layman Bernardini has recently shifted her medical practice to patient care in skilled nursing facilities across North Carolina. A Board Certified Family Medicine physician and Washington and Lee University graduate, she received her medical degree from Lake Erie College of Osteopathic Medicine. Dr. Bernardini completed her family medicine internship at Wellington Hospital in West Palm Beach, FL, and her residency at St. Luke’s Hospital in Allentown, PA. Since moving to North Carolina in 2005, she has worked in out-patient, in-patient, and skilled nursing settings. Dr. Bernardini is well-received by patients of all ages and has served as a lead provider in both major hospital networks in the Carolinas, mentoring other physicians and establishing a patient-centered home practice. In 2021, she was recognized as one of North Carolina’s top doctors in the Charlotte Region. As medical director for WoundSwift, she is dedicated to compassionate care in the Post Acute Care setting.
